If you have been pushing into the endgame of Path of Exile 2, you have probably realized that some of the coolest and most rewarding boss fights are tucked away deep inside specific league mechanics. One of those encounters is Vorana, Last to Fall. She is the faction boss for the Black Scythe Mercenaries, directly tied to Tujen and the overarching Grand Expedition questline.

You cannot stumble across Vorana while running through the normal campaign, nor will she randomly appear in your standard Atlas mapping zones. Spawning her requires a deliberate set of steps involving Expedition logbooks, specific high-tier maps, and careful explosive placement. Here is exactly how to find and spawn her.

Step 1: Complete "The Grand Expedition" Progression
Before you can even think about fighting Vorana, you need to progress through the initial stages of the Expedition endgame storyline. It is a linear chain, meaning you have to take down the first boss before she becomes available.

Talk to the NPCs: Head over to the Ruins of Kingsmarch and speak with Dannig and Makoru. This will kick off the questline and reward you with Makoru's Logbook.

Defeat Medved: Open up that initial logbook and navigate through the islands. Your goal here is to track down and defeat the first faction boss, Medved, the Fallen Seer.

Loot the Next Logbook: Once Medved bites the dust, he will drop the item you actually need: Vorana's Logbook.

Step 2: Access the Black Scythe Islands
With Vorana's Logbook in hand, you are ready to venture into the territory controlled by the Black Scythe Mercenaries. This is where things get a bit more challenging and require a specific map tier setup.

Activate the Logbook: Take Vorana's Logbook back to Kingsmarch and open it up. This unlocks a hidden network of endgame ocean islands on your expedition map.

Identify the Right Node: Carefully look over the available island nodes. You are looking for a zone explicitly controlled by the Black Scythe Mercenaries. Specifically, keep an eye out for layout nodes like the Mournful Cliffside map, which match her encounter arena.

Use a High-Tier Waystone: To actually enter and activate this specific layout, you will typically need to fuel it with a high-tier Waystone, specifically Tier 11 or higher. Make sure your character build is geared up and ready for endgame modifiers.

Step 3: Locate and Detonate the Tomb
Once you load into the map, you are not just running to an arena door. You have to actively break her out of her resting place using the classic Expedition mechanic.

Find the Boss Icon: Keep a close eye on your mini-map as you explore. Look around for a distinctive Black Skull icon. This marker shows you exactly where Vorana's underground tomb structure is located on the map.

Chain Your Explosives: Clear out the immediate area so you do not get swamped. Speak with Tujen, and then start laying down your chain of expedition explosives. You need to plan your placement so that the final blast radius directly covers Vorana's Tomb.

Trigger the Spawn: Detonate the explosives. The blast will breach the underground structure, opening up the entrance to her lair. The fight begins almost immediately once you enter, so make sure your flasks are full.

Quick Combat Tip: Watch the Dark Fog!
The moment Vorana spawns, she deploys a lethal dark fog that fills almost the entire arena. This is a brutal deathtrap. You must stay inside the tiny, safe circle directly next to her body. If you panic and step out into the black fog, you will take massive damage over time that can melt your character in just one to two seconds. Stick close, match her movements, and focus on your single-target DPS.
